Breast Augmentation is an operation that is undertaken to increase and improve the size and shape of the breast. The process involves placing a breast implant behind the breast tissue and chest muscles enabling one to get the expected size and shape.

Factors Leading To Breast Augmentation

  1. People that feel their breasts are too small and want to enlarge them
  2. Childbirth reduces the size of breasts because of breastfeeding. Thus new mothers go for this surgery to achieve desired firmness and increased size.
  3. Losing weight leads to the reduction of breast size.
  4. People that are self-conscious about their bodies, and love wearing tight fitting tops or those that wear clothes that fit around the hips but don’t fit around the bust line will go for this surgery.

Patient Candidacy

This surgery is for everyone that needs it except for a particular category of people. These are people that may have medical conditions that are considered hazardous to go through the surgery. Some of the conditions may include:

  1. Bleeding disorders
  2. Chronic diseases such as diabetes or even those suffering from HIV and Hepatitis C.

Different Types And Methods Of Breast Augmentation

  1. Saline-Filled Breast Implants

Saline-Filled Breast Implant is a kind of implant filled with saline. That sort of implant gained popularity in the past because it was considered the safest especially in the case of leaking into the body. However, this kind of implant is no longer found in Thailand as it has lost popularity as its rarely being manufactured. Patients interested in this kind of implants have to book an appointment with the surgeon 1-2 months in advance as he may need to have it imported.

  • Advantages
    – Considered to be safe as saline is not harmful to the human body
    – By adding more saline into the implants makes it possible to increase the size of the silicon.
  • Disadvantages
    – Chances of leakage are a very high thus decreasing size of the breast thus one may need another operation.
    – Implants have an unnatural feel
    – Women with thin breast tissue may experience some implant rippling on the augmented breast. The ripple may cause some sound from the undulation.
  1. Silicone Gel-Filled Implants

Unlike the saline filled silicone implants, the silicone gel-filled implants are stronger and have a more natural feel. They also have a smoother texturized surface than their saline-filled counterparts.

There are three types of silicone gel-filled implants:

A) Silicone Gel

These implants are very soft, round and have smooth, natural touch. They are movable inside the leakage part, and their probability of leakage is very high as this type of gel is highly liquid.

B) Cohesive Gel

These type of implants are also round shaped but not as soft as the cohesive gel implants. They too have a natural touch. The probability of leakage in these types of implants are lower as the liquid is quite thick.

C) High Cohesive Gel

Also known as Anatomical Silicone Implant, its known to have the least soft pad, this kind of implant still has a soft natural feel and leakage is less probable as its gel is thick in nature.

Different Shapes and Sizes Of Implants

  1. Shapes.

There are different shapes of implants that are available.

  • Round Implant.

Due to the different sizes and heights of the pads, these types of implants are round and flat at the base. They usually don’t look natural as the pad doesn’t slide on the side when someone lies down

  • Anatomical Implant

The pad on implants with this kind of shape has a sandy matte texture that helps the pad not to move because it needs to stick with the tissue that is inside the breast. The operation of this kind of shape is usually a bit more complicated than the round implant procedure as it requires the surgeon to shape and size the breast after implantation correctly.

    1. Size

    Emphasis is usually on the brand of the implant, but they usually measure between 100cc-800cc.

    Types of Surfaces of Implant Shells.

    1. Textured Implant

    These implants are thick in nature and were designed to decrease the occurrence of a condition known as Capsular Contracture where scar tissues form around the breast implants. They roll down smoothly and do not move as much as the smooth surfaced implants.

  1. Smooth Surfaced Implant

They are thinner and softer, unlike their textured counterparts. They tend to go around the breast more. Chances of fascia creation around the breast are usually evident with these types of surfaces.

A Comparison Between Smooth And Textured Surfaces

Textured surfaces implants are best for breast implantations that are placed under the breast tissue as they are highly vulnerable to contracture. Textured surfaces are designed to reduce Capsular Contracture. Smooth surface implants, however, are recommended for implants that will go under the muscle. Smooth surface implants also have a lower rate of rippling compared to the textured surface implants.

Incision types

There are two types of incisions used for breast augmentation. These are;

  • A hidden wound under the fold of the armpit which is the most popular and is usually 2-3 centimeters long
  • Making a cut around the base of the nipple especially for people with a big enough nipple base which should measure about 3centimetres.

Types of Implant Placement

  1. Submuscular Placement

Submuscular Placement is the process of placing the implant under one big breast muscle where the muscle presses on the tip of the implant. The tissue supports the implant such that there is no implant movement. This type of implant placement makes the breast look natural.

  1. Sub-glandular placement

This technique occurs when the implant goes under the breast above the chest muscle. If expertly done it can take a very short time. Sportswomen are more viable for this technique as they are known to have a lot of tissue between the implant and the muscle.

Pre-Operation
  1. Pre-operation Consultation
  • Depending on the breast tissue the patient and surgeon make decisions based on what the patient expects and his general health
  • The doctor discusses implant size, texture, shape, placement and incision site during this consultation.
  • Patients will be advised not to take medications like aspirin, ibuprofen or herbs 2weeks before the surgery.
  • The doctor discusses the technique he will use on the patient.
The Procedure

The surgery will take around one to two hours. The doctor will make a cut at the agreed position where there will be a creation of a  “pocket.” The implant will then be put in the pocket and adjusted to ensure that it fits perfectly. The surgeon then cleans the wound at this point. Tubes will be used to drain the surgical site after completion of the breast augmentation operation. Finally, the surgeon stitches the wound and covers the chest area with some elastic bandage which helps in reducing blood circulation and stabilizing the position of the implant. The healing process starts from here where the patient will experience some pain for the first 48 hours, but they will get painkillers to help with the pain.

Post Operation
  1. The wound
  •  The patient is advised to avoid showering, and only sponge bathe.
  • The surgeon removes stitches from the wound after five days after which the patient is instructed to clean it 1-2 times a day.
  • Once the wound heals, scar removal cream can be used to remove the scar
  1. Sleeping position

The patient is advised to rest on their back after post breast augmentation operation so as to ensure the implants are in perfect position especially during the initial days of healing.

  1. Diet

Patients should eat a well-balanced diet and not preserved foods and also avoid drinking and smoking. They should take medications prescribed as instructed by surgeon

  1. Intercourse

Patients can have sex as long as they keep off the breasts during the first week

  1. Side Effects

Patients will experience some soreness, breast bruising, a bit of swelling and some breast sensitivity.

  1. Dressing

Dressing regards the type of bra that you wear post operation. Patients are advised to wear bras all the time, day and night, especially for the first two weeks. The bra helps keep the implants and breast in perfect position. After the two weeks, you can be able to sleep without a bra.
