Tear troughs, also referred to as nasojugal grooves, are natural depressions below the eye where the tears drain through.
As we age, their appearance often becomes more pronounced, which can create dark circles under the eyes, causing the eyes look sunken and hollow.
This occurs more commonly in people with thinner faces, although hereditary and environmental factors can also play a role in their formation.

Tear trough surgery with injectable dermal fillers is a simple, non-invasive procedure performed on an outpatient basis. The procedure involves injecting the treatment area with hyaluronic acid, which is a linear polysaccharide found naturally in the human body.
Hyaluronic acid protects the skin which reduces wrinkling, thus restoring a youthful and rejuvenated look to the under-eye skin. Tear-trough treatment with dermal fillers can deliver soft and natural results that last 6-12 months depending on the filler. Recovery is also faster than in the fat transfer procedure, with the main side effect being mild bruising, which typically resolves within 2-3 days.
Fat transfer
As an alternative to dermal fillers, the tear trough area can also be treated using autologous fat grafting.
Fat grafting provides a long-lasting solution for hollows eyes and under-eye bags. The process involves extracting fat from the patient’s own body (typically the thighs, buttocks or abdomen) with a liposuction cannula, purifying it and extracting it into the tear troughs to reduce bags under the eyes.
Since you’re using your own transferred fat, there is virtually no possibility of your body rejecting the fat, and the results last at least a few years.

Before the Surgery

- Wash your face thoroughly before surgery and don’t apply any makeup
- Wear glasses instead of contact lenses
- Plan to take 5 days off work and your normal activities
- Make sure you have someone to drive you home after surgery
- You may want to prepare sunglasses to wear afterwards
Recovery
- Apply a cold compress to reduce swelling after the first 2 days
- Sleep with your head elevated
- Keep the operative area clean and dry and take all the medications prescribed by your doctor
